    I tried to respond but got stuck. I strongly suggest you revise your questionnaire as many questions are unanswerable or the respondent will need to make assumptions bout what you mean.

    I agree to the comment above. The questions are not properly designed and the results will be very unreliable. Most of the answers will be subjective and assumptions which will be useless in scientific analysis.

  3. Try to revise your questions, state the objective of your survey clearly as you are collecting personal information, state briefly the categories of respondents that you expect...are they healthcare professionals, government officials or what?

    The survey is somehow confusing as you have posted online and you might end up having responses from unqualified respondents which will result into unusable data

  4. There are certain things in your questionnaire which are not clear and needs some revisions.

    1.The objective of the survey, is it for Masters, PhD etc

    2. You are collecting personal information, are you going to use any of the personal information collected, and for what purpose?

    3. Who are the respondents of your survey? Are you expecting responses from government officials, healthcare professionals, or health informatics individuals?

    If they are healthcare professionals, is this the right forum for distribution of your questionnaire?

    How will you monitor that you have responses from only those qualified? Trust me, you might end up having responses from those who have just completed one semester in college regarding Data Mining who assume they are the experts of the area.
