On 12 April 2010, the Dar es Salaam regional Commissioner Mr. William Lukuvi announced throughmedia that 96 health workers at Mwananyamala Hospital have been transferred to other working stations, majority within Dar es Salaam region. He attributed the transfer to the longstanding poor health care service provision at the hospital.

We doubt if mere shuffling of staff around Dar es Salaam region is the right solution to those problems. Citizens are experiencing same problems in almost all public facilities in Dar es Salaam region. Historically, transfer would usually spread the vices Again, this particular transfer seem to have punished bad and good workers, which is unjust, and therefore an indication of a weak management.

Service users have complained about poor quality service delivery at Mwananyamala Hospital (and other public health facilities) for years. Complaints are usually regarding malpractices, negligence, unfriendly attitude of service providers, unavailability of medicine and diagnostic equipment, bribery, overcrowding, and long waiting period to be served.

We would like to propose some measures to address this chronic problem not only at Mwananyamala Hospital but all health facilities across the country.
First, we expect strengthened hospital management and administration including corrective action against health workers who maltreat patients and whose performance is unsatisfactory. This may include firing on chronic behavior instead of transferring.

Second, ensure timely availability of quality working tools including medicines, equipment and supplies. Some available working tools are as good as no tools. Third, ensure adequate space and beds for pregnant women before and after delivery.

The fourth measure is to improve pay and benefits of all cadres at health facilities. While some health workers are entitled to some kind of financial allowances, many are not. This leads to inequality in pay and demoralization of especially the frontline workers.

The fifth measure, which is very important in our opinion is to make sure that, implementation plans and budgets, income and expenditures are transparent to both health workers and service users and are publicly available. It is not enough to say that the hospital does not have enough budgets to deliver services. How much is not enough, and how have we prioritized what we have?

  2. Ni kweli sio ufumbuzi wa moja kwa moja aliofanya mkuu huyu, lakini naamini ni jitihada mojawapo ya kutatua tatizo.
    SABABU:
    1.Hao waliohamishwa tayari wanajua kwamba wanatakiwa wajirekebishe.
    2.Sehamu watakazohamia kama zinautendaji mzuri itabidi wafuate.kama methali yetu inavyosema ukienda ugenini inabidi ufufuate jadi zao,kama wanalala chini nawe itabidi ulale chini.
    3.Ukweli ni kwamba Tanzania bado tuna upungufu mkubwa wa wafanyakazi katika idara ya afya, hivyo isingewezekana kabisa kuwafukuza.Taifa limewasomesha hawa watu kwa hela nyingi pia(cost benefit analysis)

  3. Je hii ya kuhamisha imefanyiwa kazi kwa kina? Sasa matatizo yamesambazwa nchi nzima. Ni kama wanafunzi wamehitimu sasa wanapewa vituo vya kazi wakafundishe wanafunzi. Nina maana kuwa hawa sasa nao wanakwenda kuwaaribu wenzao.Ukweli ni kuwa hata sehemu hospital nyingine matatizo yapo japo yalikuwa si kama mwananyamala ambapo yalikuwa ya waziwazi, sasa ukiongezea hawa wasi hata na aibu hali itakuwa tete kote walipo.

    Swala lingine, je uongozi nao umebadilishwa. Matatizo mengine yanaletwa na uongozi mbaya, na hilo ni lazima lilikuwa ni tatizo.

  4. Katika masuala ya uongozi na utawala huwa hakuna namna fulani ya kutatua tatizo ambao inaweza kutumika kwa kila tatizo la namna hio linapotokea. Hoja ya kuwa kuwahamisha si sahihi, me naipinga nasema kwa kiasi fulani itapunguza tatizo. Kuna kitu kinaitwa "organization culture", kwa vyovyote vile hao waliohamishwa ni ngumu sana kwenda nayo huko walikopelekwa coz nako huko wana organization culture yakwao. Hiyvo basi huyo anaetoka M'nyamala culture yake itamezwa katika hizo walizokuta huko. Pia kutoka na hali ya nchi yetu pendekezo no.2 ni ngumu kutekelezeka kutokana na urasimu wa kiserikali unaozuia vifaa vya afya kutolewa vya kutosha na katika muda muafaka. Hili ni tatizo la kimfumo kiasi kwamba Lukuvi kama mkuu wa kasehemu ka Tanzania hana nguvu ya kubadili mfumo kupunguza urasimu. Kwa kuwa suala la kurekebisha mfumo ni suala la muda mrefu, anachoweza kufanya Lukuvi ni kuchukua hatua za haraka "short term or ad-hoc soulutions" zitazoweza kutatua tatizo kwa sasa kwa kiasi fulani japo si tatizo lote, wakati haya mengine yanatatiwa majibu taratibu katia muda unaofaa. Mimi binafsi nampongeza Lukuvi na team yake kwa hatua hiyo, kwa kuwa mapendekezo yaliyotolewa na mdau Irenei hayatekelezeki kwa hali ya sasa ya ki JK. Bora reshufle ifanyike kuokoa maisha ya ndugu zetu yanayoteketea M'nyamala hosp. Aksante.

    We thank you for bringing this affair on the stall. The recommendations brought for improving health services delivery in Tanzania are reasonably clear and definitely are within the capacity of the country which says health sector is a precedence (mh I guess this is purely a definition of political priorities in the ears of underprivileged and humble Tanzanians). Personally, I don’t see the logic behind whereas awkward spending on tax payer’s money on superfluous public expenditures such as government printing of calendars (what are they advertising as the Government? What are they selling? Whose money is it used for such spending? Traveling abroad and within the country with limitless allowances and per Diems which ended in un-fair accumulation of wealth by civil servants accompanied by purchases of extra ordinary luxurious 4x4 whilst leaving behind health facilities with no skilled attendants and always prescription are subjected to pain killers….this is insane! Strategic policies are the out cry of everybody in this country. Our shelves are full of good policies but there is NO BODY enchanting trouble to ensure that they are implemented. Just coming from NOWHERE and start relocating health workers? Is this a strategic resolution?
