It is either President Jakaya Kikwete or Dr.Jakaya Kikwete, not President Dr. Jakaya Kikwete. If you want to use multiple titles, then you should include all of his titles to have something like President Col. Dr. Jakaya Kikwete, CCM Chairman and Command in Chief of Armed Forces. If the doctorate title is so important that it must appear, then change the way you address him to be something like, the President of Tanzania, Dr Jakaya Kikwete.
